I have interpreted “decommissioned” to mean withdrawn from service, in which case the following Royal Navy and Royal Fleet Auxiliary vessels have been decommissioned in the last five years:

2012

HMS TURBULENT

HMS YORK

HMS LIVERPOOL

2013

HMS EDINBURGH

2014

HMS TIRELESS

HMS ILLUSTRIOUS

2015

RFA ORANGELEAF

2016

RFA BLACK ROVER

RFA DILIGENCE
